Declaration - please read in full
The club’s Code of Conduct for both swimmers and parents/ guardians and the club’s constitution can be found on the club website.
*I acknowledge that I have been directed to, and given the opportunity to read, the constitution
of Camelford stingers Club and the Codes of Conduct.
*I confirm my understanding and acceptance that the constitution and Codes of Conduct (both as amended from time to time) shall govern membership of the Club. I further acknowledge and accept the responsibilities of membership upon members as set out in the constitution.
*I agree to abide by the Codes of Conduct and to do my best to ensure the swimmer(s) in my care abide by them.
*I agree that the Club may send me documents and information in electronic, rather than physical, form.
*I agree to pay fees monthly in advance by standing order unless other arrangements are made and to pay any other charges in full on demand.
*I agree to update the club without delay of any changes to my contact details.
*I give permission for the coach or team manager or other person accompanying my child/myself to give any immediate necessary medical treatment as
directed by medical authorities.
*I give permission for the club to use photos of my child at events/training for social media (please let our welfare officer know if you do not want to allow this)
